How to upload your immunization documents


Submit your immunization information by following the steps below – also available as a PDF

  1. Access the secure web portal by visiting commonwealthu.edu/HealthForms
  2. Sign in using your CU email and password
  3. Enter your date of birth
  4. View the Web Portal Menu. Signing in from a phone? Click on the   (top left)
  5. Click on "Health Forms" to enter immunization dates

Questions? Click on "Messages" and choose your home campus to send a secure message to the Health Services staff. We are here to assist!
